Plate 215mm diameter with grey border split into three segments. In centre picture of church and below Holinshead Street United Reformed Church, chorley. On back is written "The Church was built in 1792 as New Street Independent Chapel, but it was not until 1824 that it became Hollinshead Street Congregational Chapel. In 1972 it became a member of the United Reformed Church upon the union of the Prebyterian Church of England with the Congregational Church in England and Wales. The Churches of Christ joined in 1981".
